Accepted Students Forms and Information
This section only pertains to participants who have been accepted and are attending the Institute.
Each TFAS International Institute requires participants to complete pre-institute paperwork that ensures prompt payment and services before and during the Institute. Please be sure to read the description and directions about the forms before completing them. You are not required to submit every form unless requested by TFAS.
EJI also requires that you send one photo electronically for our program booklet. This photo should be similar to standard passport size of just yourself. Please send this scanned picture to Ms. Brigit Moore, International Programs Assistant.
1. Attendance Agreement: The Attendance Agreement is a contract signed by the participant that clearly defines the expectations of each participant during the Institute. It is necessary that this form be completed and sent back to the program administration prior to the student leaving for the Institute.Form must be submitted by all participants.
2. Travel Form: The Travel Form provides TFAS with the pertent travel information of each participant. This information is necessary in case any issues arrive during travel or if extra accomodation is needed due to visa restrictions. This form should be completed before the participant leaves for the Institute. We understand that travel details take some time to confirm so it is possible to delay this form until your travel reservations have been confirmed.
3. Payment Information Form: (It is not necessary to complete this form unless it is requested by TFAS)The Payment Information Forms indicates the methods of payment TFAS will accept from each participant. Payment for the Institute can be accepted in three ways. Consult the payment form on specific instructions for each method.
